Not your resume. The person behind it. Who are you at this stage of your life?

02What is bringing you to this experience now?

Why this season? Why this moment?

03What feels most alive in your life right now?

What is energizing, expanding, or calling to you?

04What feels most challenging right now?

Where do you feel stuck, uncertain, disconnected, or in transition?

05What are you hoping to receive from this experience?

What would make this year meaningful for you?

06Tell us about your relationship with personal growth.

What forms of inner work, therapy, coaching, spiritual practice, learning, or self-reflection have been most impactful in your life?

07Tell us about your experience in community.

What has helped you feel connected, supported, and seen by others? What has made that difficult?

08What do you believe you would contribute to this community?

09What does commitment mean to you?

What would it look like for you to fully engage in a year-long experience?

10Is there anything else you would like us to know?

Health, Support & Readiness

This section helps us understand your current wellbeing and readiness so we can support your participation thoughtfully. Your responses are held with care and complete confidentiality.

Physical Health

Do you have any current medical conditions, recent surgeries or injuries, or physical considerations we should be aware of?

This includes anything that may affect your participation in movement practices, breathwork, travel, or retreat activities such as sauna or cold plunge experiences.

Emotional & Mental Health

Have you experienced any of the following at any point in your life?

Select all that apply. This information helps us understand your history and provide appropriate support.

Are any of these concerns currently active in your life?

If yes, please share briefly. You don't need to share more than you're comfortable with.

Your Support System

Are you currently working with a therapist, psychiatrist, physician, coach, spiritual mentor, or other support professional?

Please share as much or as little as you'd like.

What practices or forms of support help you stay grounded and regulated in your daily life?

Readiness & Community

What draws you to this experience at this particular moment in your life?

What challenges are you currently navigating?

You might be in a season of transition, loss, growth, or uncertainty — share whatever feels true.

What kind of support do you hope to receive from a community of women?

What do you feel you have to offer this community?

Stability & Capacity

Are there any circumstances in your life right now that may significantly affect your ability to participate consistently throughout the year?

Major transitions, caregiving responsibilities, health concerns, work demands, or anything else worth naming.

How do you feel about your current emotional capacity to engage in a year-long, growth-oriented community?

We are not looking for a perfect answer — we are looking for honest self-awareness.
